You must provide two different current utility bills (electric, water, gas, land phone) that clearly show your name, street address, service address and date.  No cell phone bills will be accepted to verify residency.  Any document with a post office box as an address will not be accepted, unless the service address is listed.

IF YOU LEASE YOUR HOME:        

  1. You must submit a current lease agreement signed by the homeowner. (all occupants must be listed on the lease)
  2. You will also need to provide two different current utility bills (electric, water, gas, land phone) that clearly show the street address and name shown on the lease.

 IF YOU HAVE MOVED (to a new house in our zone):

  1. You must bring a copy of your new deed or lease.( all occupants must be listed on the  lease)
  2. You must also complete a new affidavit of legal residence in the school office. 
  3. Two different current utility bills (electric, water, gas, land phone) which clearly show the street address and name shown on the lease must also be provided.
